Payment & Cancellation Terms

1. Monthly program pricing

• In-clinic program: $179 per month.
• Telehealth program: $199 per month, including shipping.
• This is an ongoing monthly price, not an introductory rate. No annual prepayment or multi-month commitment is required.
• Your monthly price stays the same if your prescribed dose level changes.

2. Eligibility comes first

Booking a consultation does not guarantee a prescription or enroll you in the program. Your provider will determine whether treatment is medically appropriate. You are not charged the monthly program price unless you are approved and choose to enroll.

3. Billing

• Your first monthly payment is due when you enroll, before your first medication is provided.
• After that, your card on file is charged automatically on the same date each month (your billing date) until you cancel.
• If a payment fails, we will contact you. Medication and services may be paused until your account is current.

4. Cancelling your program

• The program is month to month. You can cancel at any time by calling or visiting the clinic.
• To avoid your next monthly charge, cancel before your next billing date.
• After you cancel, you keep access to your program through the end of the month you have already paid for.

5. Refunds

Because medication is prescribed and prepared for you individually, monthly payments are generally non-refundable once medication has been dispensed or shipped. If you are charged and no medication or services have been provided for that month, contact us and we will review a refund.

6. Rescheduling and missed appointments

Please let us know as early as possible if you need to reschedule a visit, so we can offer the time to another patient. Frequent missed appointments may delay your care, since your provider needs to see you to continue treatment safely.

7. Changes to these terms

We may update these terms from time to time. We will tell current patients about any price change before it takes effect.
